Job Summary:
Regional Health Specialist role is integral to the Care Management department, providing key support to Care Managers in delivering appropriate services to meet members’ needs. A Regional Health Specialist assists Care Managers with their duties, augmenting the team’s workforce capacity and delivering most Tailored Care Management member required contacts. This work may include meeting with members virtually or in person, recording member health updates, suggesting changes to the Care Manager for current care plan efficacy, and assisting the member in finding additional medical or community-based resources, among others. In addition to supporting member outreach/engagement, education, and referral services, Regional Health Specialist must also collaborate effectively with the care team to prevent duplicative efforts and communicate member updates.
